About the position
The Resident Service Coordinator plays a crucial role in facilitating and implementing core programs for residents in supportive housing. This position focuses on identifying community service needs, developing strategies to connect residents with necessary services, and building partnerships to enhance resident stability.
Responsibilities
- Facilitate and implement core programs as delegated by the program model.
- Collect and record data to measure program outcomes.
- Identify human and community service needs of residents.
- Develop strategies to link residents with needed services, including assistance in overcoming barriers to access.
- Build community partnerships that bring services and actively participate with other staff toward resident stability.
Requirements
- 2 years of relevant case management experience in supportive housing.
