About the position

The Resident Services Coordinator (RSC) plays a crucial role in enhancing housing success for residents, particularly the elderly, individuals with disabilities, and low-income families. This position focuses on assessing resident needs and linking them to supportive services and community resources, ensuring compliance with HIPAA regulations. The RSC will support two communities, Redwood Shores and St Joseph, by providing non-clinical case management and organizing wellness programs.

Responsibilities

  • Provide non-clinical case management and referral services to all resident sites, securing necessary social services for residents.
  • Assess and identify resident needs, develop and implement individual healthy aging plans, and educate residents on self-management of chronic conditions.
  • Conduct initial assessments within 30 days of move-in and update assessments as needed, including annual updates on Activities of Daily Living (ADL).
  • Utilize and maintain accurate documentation through AASC On-Line.
  • Organize and coordinate on-site wellness and health improvement programs, events, and activities.
  • Produce and submit weekly reports to the housing administrator and supervisor.
  • Conduct outreach and engagement, including home visits when necessary.
  • Collaborate with the Activities Coordinator to develop community-building activities for residents.
  • Establish volunteer support programs with community volunteers.
  • Maintain accurate documentation on resident demographics, issues, requests, and interactions through online software.
  • Attend staff meetings and ongoing training or conferences for Resident Services Coordinators.
  • Assist residents in identifying and accessing needed services and benefits, acting as a liaison or advocate when necessary.
  • Maintain resident confidentiality according to established guidelines.

Requirements

  • Experience in case management or social services, particularly with elderly or disabled populations.
  • Strong communication and interpersonal skills to build relationships with residents and service providers.
  • Ability to assess needs and develop individualized plans for residents.
  • Familiarity with community resources and services available to residents.
  • Proficiency in using online documentation systems.

Nice-to-haves

  • Experience working in a residential community setting.
