About the position

The Resident Services Coordinator is responsible for providing outreach, case finding, assessments, and care planning for seniors, disabled adults, and families within the Resident Services Program. This role focuses on addressing the psychosocial, health, financial, recreational, nutritional, and legal needs of clients, ensuring they receive the necessary support and resources.

Responsibilities

  • Establish and maintain relationships with individual residents, their families, and neighborhood groups.
  • Identify and document residents in need of services and maintain awareness of health status.
  • Serve as a liaison or advocate with outside social and health agencies for the residents.
  • Provide individual casework and case management for frail or at-risk residents, including assessment, development of individual care plans, coordination of services, and monitoring.
  • Provide information and referral, crisis intervention, and counseling with individuals and groups as needed.
  • Maintain accurate and up-to-date records and files on all clients receiving casework services.
  • Identify barriers to service provision and effectively problem-solve to resolve these issues.
  • Facilitate and support 'neighbor-to-neighbor' family and volunteer caregiving, including direct responsibility for designated volunteer programs at specific sites.
  • Provide resident population with current information on services, benefit programs, and other resources.
  • Identify and develop training opportunities for resident groups and site staff.
  • Work cooperatively with housing manager or agency director to address needs of residents.
  • Ensure completion of required documentation and statistics.
  • Complete training related to distribution and administration of Narcan.
  • Perform other duties as assigned.

Requirements

  • Bachelor's degree in Social Work, Sociology, Gerontology, Psychology, or related field required.
  • Minimum of three (3) years of recent experience in a social services capacity.
  • Demonstrated proficient interpersonal, verbal, and written English communication skills.
  • Ability to relate appropriately and effectively to residents and families, to other staff and to personnel of other community agencies.
  • Ability to work independently and as a member of an interdisciplinary team.
  • Proficient with Microsoft Office suite of products (Word, Excel, Outlook).
  • Ability to travel to multiple locations as needed.
  • California driver's license, proof of insurance, and a clean driving record required.

Nice-to-haves

  • Master's degree in related field preferred.
  • Bilingual capacity may be required for some specific sites.
