About the position

The Resident Services Coordinator plays a vital role in supporting vulnerable disabled Veterans by synthesizing agency and community resources to help them achieve lasting recovery from homelessness through supportive housing. This position involves collaborating with residents, nursing staff, social workers, and family members to ensure the highest quality of care and effective management of care plans in nursing home and memory care facilities.

Responsibilities

  • Engage with residents to collaboratively complete the initial assessment and develop and implement individualized Housing Stability Plans (HSP) outlining short term and long-term goals.
  • Provide services utilizing evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, Trauma Informed Care, Critical Time Intervention and Housing First Practices.
  • Coordinate with community providers to offer additional services in housing stabilization, community integration, and referrals for substance use, primary and mental health care.
  • Facilitate/teach daily living skills and workshops for residents in groups and individually as outlined on each resident's HSP.
  • Conduct scheduled home visits with each resident at the frequency determined in collaboration with the Program Manager.
  • Provide crisis intervention as needed under the supervision of the Program Manager.
  • Maintain all client records and information in accordance with internal policies and ensure compliance with HMIS.
  • Complete all documentation in a timely and efficient manner.
  • Attend scheduled workshops, trainings and meetings as required.
  • Adjust priorities and schedules to meet specific customer needs.

Requirements

  • Bachelor's degree in social work, mental health, psychology, or related field required.
  • Minimum of three years related field experience (experience negotiable for the right candidate).
  • Ability to establish cooperative working relationships with clients, colleagues, and volunteers.
  • Ability to manage crisis situations.
  • Personal car and valid North Carolina driver's license (no transporting clients in personal vehicle).
  • Acceptable annual Motor Vehicle Record check.
  • Ability to pass a Level II Background Screening.

Nice-to-haves

  • Experience in homeless services for 2 years or more.
