About the position

The Resident Services Coordinator will provide direct supportive services, employment and training support, and appropriate referrals for residents at Carrfour Supportive Housing Programs. This role focuses on engaging with residents to develop and implement individualized Housing Stability Plans, utilizing evidence-based practices to promote self-sufficiency and housing stability.

Responsibilities

  • Engage with residents to collaboratively complete the initial assessment and develop individualized Housing Stability Plans (HSP).
  • Provide services utilizing evidence-based practices such as intensive case management, Motivational Interviewing, and Trauma Informed Care.
  • Coordinate with community providers to offer additional services in housing stabilization, money management, employment and training, and mental health care.
  • Facilitate and teach daily living skills and workshops for residents.
  • Conduct scheduled home visits with each resident as determined in collaboration with the Program Supervisor.
  • Work with the property manager to establish community building activities and facilitate workshops.
  • Provide crisis intervention as needed under supervision.
  • Maintain client records in accordance with policies and ensure compliance with HMIS.
  • Complete all documentation and paperwork in a timely manner.
  • Participate in quarterly staffing to address resident progress towards HSP goals.
  • Attend scheduled workshops, trainings, and meetings as required.

Requirements

  • High School Diploma or GED required.
  • Experience working directly with individuals from diverse racial, ethnic, and socioeconomic backgrounds.
  • Understanding of working directly with formerly homeless individuals and at-risk populations.
  • Proficient in Excel, Adobe, Microsoft programs, and database applications.
  • Must have a valid driver's license and reliable transportation.

Nice-to-haves

  • Higher education in social work, mental health, psychology, or related field preferred.
  • Bilingual (English/Spanish or Creole/English) preferred.
  • Experience in social work and case management (2 years preferred).
