About the position

The Providence Center is seeking a Residential Case Manager for Brick Manor on a per diem basis. This role involves providing clinical case management services to facilitate program goals and objectives, focusing on a person-centered approach to address mental health symptoms and co-occurring disorders. The case manager will work with consumers to help them achieve the highest level of independence possible.

Responsibilities

  • Provide case management and support to consumers.
  • Treat consumers with respect.
  • Provide support counseling, problem solving, contracts, and limit setting.
  • Establish attainable goals with the consumer.
  • Involve consumer, agencies, and others in treatment planning.
  • Collaborate with the clinical team on engagement insight and needed supports.
  • Provide support counseling for mental health issues, substance use disorders, and medical conditions.
  • Assist consumers in deciding which areas they want to address.
  • Assist and monitor group home consumers with medical interventions and hygiene needs.
  • Encourage consumers to attain the highest possible levels of independence.
  • Accept the values and lifestyles of consumers.
  • Use a flexible approach with a diverse population.
  • Make assessments and provide appropriate crisis interventions and risk management as needed.
  • Teach consumers adult daily living skills in the group home and supervised apartments.
  • Initiate treatment and advocate for consumers within the network of care.
  • Assist in the development and implementation of socialization and recreational activities.
  • Transport consumers to planned activities and appointments.
  • Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements.
  • Maintain cooperative relationships with Center staff, consumers, community agencies, and the public.
  • Serve on appropriate Center committees.
  • Work flexible hours, including evenings, weekends, holidays, and during inclement weather.

Requirements

  • High School Diploma/GED and/or at least 5 years of experience in a behavioral health setting.
  • Experience in residential facilities is helpful.
  • Must have a valid driver's license and insured auto.
  • Must be able to work flexible hours including evenings, overnights, weekends, and holidays.
