About the position

The Residential Case Manager is a part-time position responsible for providing assertive outreach and support services to homeless adults with behavioral health issues. This role involves facilitating program admissions and discharges, monitoring client eligibility, and ensuring appropriate service linkages. The Case Manager will work closely with clients to promote community involvement and independence while maintaining their health and safety.

Responsibilities

  • Develop relationships with clients and encourage community involvement and independence.
  • Ensure the health, safety, and well-being of clients by adhering to health and safety policies.
  • Meet visitation and treatment team compliance goals as directed by the Team Lead.
  • Complete and submit necessary forms for client admissions and documentation in a timely manner.
  • Attend treatment team meetings and residential house meetings as applicable.
  • Trigger immediate increases in services for clients experiencing crises.
  • Provide interim care for clients waiting for higher-level services.
  • Coordinate benefits application processes with state service centers and other agencies.
  • Act as an ombudsman for community and state service providers, addressing suggestions and complaints.
  • Work independently and respond to crises on-call when assigned.
  • Facilitate transitions of clients to and from various care levels as appropriate.

Requirements

  • Bachelor's degree in psychology, counseling, human services, social work, nursing, rehabilitation, or related field from an accredited college or university.
  • Three years of clinical/administrative experience in providing support to individuals with severe and persistent mental illness.
  • CPR/First Aid Certification.
  • Crisis Prevention and Intervention Training.
  • Assistance with Self-Administration of Medication certification.

  • Training in PM-46 and reporting abuse, neglect, and mistreatment.
