About the position

The Retail Health Coordinator at CVS Health plays a crucial role in enhancing patient care by coordinating healthcare services and addressing health care gaps for members. This position focuses on delivering excellent customer service while managing patient interactions and care plans, ensuring that patients receive the necessary support and resources to improve their health outcomes. The coordinator works under the supervision of clinical leaders and collaborates with various healthcare teams to facilitate effective communication and care delivery.

Responsibilities

  • Coordinates patient flow throughout all scheduled appointments
  • Interacts with customers/patients/members/coworkers telephonically and through technology-based interactions
  • Schedules, tracks, and reconciles patient referrals and diagnostic tests
  • Demonstrates proficiency managing email and using remote communication software
  • Adheres to care management, privacy and confidentiality, and quality management processes
  • Attends weekly team huddles to stay connected with the Retail Health field
  • Provides superior customer service to internal and external clients
  • Communicates with clinical team, patients, and families on recommended patient care plans
  • Assists with completing and tracking patient forms and letters
  • Notifies clinical care team of any changes or barriers encountered
  • Cross trains to other areas of retail health management as appropriate
  • Orientation and training of staff as assigned
  • Demonstrates respect and regard for the dignity of all patients, families, and fellow employees

Requirements

  • High School Diploma or equivalent required; Associate degree preferred
  • 3-5 years of healthcare, customer service, patient relations, or hospitality experience
  • Strong computer, data entry, and typing skills
  • Proficiency in Microsoft Office applications, including Outlook, Word, and Excel
  • Strong organization skills and attention to detail
  • Ability to multitask and prioritize while maintaining a pleasant demeanor
  • Self-starter with the ability to work with minimal supervision
  • Excellent customer service and communication skills

Nice-to-haves

  • Certification/Licensure: Medical Assistant preferred
  • Experience in a call center or telephonic care management setting preferred
  • Bilingual (Spanish) preferred
  • Experience with electronic medical records, e.g., Epic preferred
  • Prior experience handling sensitive information confidentially
