About the position
The Home Health RN Case Manager at Interim HealthCare is responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services within the Greater Seacoast community. This role offers flexibility in scheduling, allowing nurses to balance their professional and personal lives effectively. The position is available in full-time, part-time, and per diem capacities, catering to various work preferences.
Responsibilities
- Manage a defined caseload of patients in the community.
- Provide direct patient care and comprehensive nursing services.
- Coordinate with healthcare professionals to ensure quality patient care.
- Document patient progress and maintain accurate records.
- Educate patients and their families on health management and care plans.
Requirements
- Valid Nursing License in the State of New Hampshire.
- Nursing experience required.
- Current CPR Certification required.
- Reliable transportation.
