About the position

The RN, Abdominal Transplant Coordinator is responsible for the initial assessment of transplant evaluation patients, assisting them throughout the transplant process, and providing follow-up care post-transplant. This role emphasizes high-quality, efficient care while ensuring exceptional experiences for patients and their families. The position requires a commitment to safety, compassion, and collaboration within a healthcare team.

Responsibilities

  • Perform initial assessment of patients' biophysical and psychosocial status.
  • Document the nursing process, including problem list, plan of care, and patient education.
  • Provide appropriate pre/post-transplant interventions within limits of licensure and/or scope of practice.
  • Evaluate and collaborate with other healthcare providers to achieve patient outcomes.
  • Plan, coordinate, and organize the discharge plan with inpatient nursing staff.
  • Direct and provide guidance to staff, case managers, and physicians.
  • Ensure safe care to patients, staff, and visitors by adhering to policies and procedures.
  • Promote individual professional growth and development through continuing education and skills competency.
  • Serve as a preceptor, mentor, and resource to less experienced staff.

Requirements

  • Bachelor of Science in Nursing (BSN) preferred.
  • Registered Nurse with a current State of Texas license to practice professional nursing.
  • Two (2) years of clinical experience, preferably in critical care or transplant coordination.
  • Experience in transplant certifications is preferred.

Nice-to-haves

  • Solid ICU experience is preferred.
