About the position

The Acute Care Manager is responsible for coordinating and managing patient care to meet their diverse needs, ensuring continuity of services, and reducing fragmentation in care. This role involves collaboration with interprofessional teams to enhance communication across various stages of care, from acute to post-acute settings.

Responsibilities

• Ensure appropriate clinical documentation to support the level of care.
• Analyze the patient's clinical status, treatment plan, and medical history to identify documentation gaps.
• Identify principal diagnoses, secondary conditions, co-morbidities, complications, and procedures documented in medical records.
• Serve as a consultant and educator to medical staff regarding patient status, placement, payer requirements, and resource utilization.
• Assure appropriate sequencing of care and interventions for optimal results and smooth transitions in hospital care.
• Proactively identify and remove barriers that impede the progress of care.
• Coordinate level of care appropriateness with admission status determination.
• Ensure clear admission orders and act on cases not meeting admission criteria.
• Anticipate principal diagnoses and procedures.
• Maintain knowledge of healthcare economics, trends, and reimbursement methodologies to apply in daily practice.
• Recognize situations requiring immediate intervention and act accordingly.
• Continue professional development and apply necessary knowledge and skills for patient care.

Requirements

• Bachelor's Degree in nursing preferred.
• Minimum of 3 years clinical experience.
• 3-5 years of direct patient care or case management experience in a similar hospital setting preferred.
• Ability to analyze clinical, financial, and social situations and direct the healthcare team.
