Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts initial assessment, re-assessment, and focused assessment based on patient condition, setting, and population.
- Identifies problems based on clinical judgment about the patient's response to health conditions or needs.
- Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family and formulates a plan of care.
- Implements nursing care according to the care plan, ensuring continuity of care during hospitalization and in preparation for discharge.
- Ensures patient safety, administers medication, provides ordered treatments, and educates patients and families.
- Documents care in the patient's healthcare record.
- Continuously evaluates the patient's status and the effectiveness of nursing care, modifying the care plan as needed.
- Coordinates care by managing and correcting the sequence, timing, and effectiveness of patient care from admission to discharge.
Requirements
- Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days.
- Basic Life Support (BLS) certification from the American Heart Association.
- Graduation from an accredited School of Nursing.
Nice-to-haves
- Advanced Cardiac Life Support (ACLS) certification from the American Heart Association.
- Pediatric Advanced Life Support (PALS) certification.
- Neonatal Resuscitation Program (NRP) certification.
- Bachelor of Science in Nursing.
