About the position

The RN Admission Care Liaison is a critical role within Ohio's Hospice, responsible for providing quality nursing care and coordinating hospice services for patients at the end of life. This position involves admitting patients into the hospice program, ensuring they and their families receive the necessary resources and support. The role requires effective documentation and a commitment to maintaining a high conversion rate of admissions.

Responsibilities

  • Admit patients into the hospice program efficiently.
  • Provide patients and families with appropriate resources needed at the end of life.
  • Maintain a conversion rate of 75%.
  • Document services and care accurately using electronic medical records.
  • Drive to patient locations during various weather conditions.

Requirements

  • Graduate of an accredited nursing school with an Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) preferred.
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years of acute care nursing experience preferred.
  • CPR Certified.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
  • Sufficient computer skills for documentation purposes.
  • Valid Driver's License with a safe driving record.

Nice-to-haves

  • Experience in hospice or palliative care.
  • Certification in Hospice & Palliative Medicine (CHPN) within two years of employment.
