About the position

The RN Admission/Discharge Coordinator is responsible for assessing, planning, implementing, and evaluating nursing care for all admissions and discharges at the facility. This role ensures continuity of care by providing direct nursing care and guiding the care provided by other staff members. The coordinator also participates in resident and family education, advocates for residents' rights, and collaborates with the interdisciplinary care team to meet the needs of residents effectively.

Responsibilities

  • Assess residents on admission, readmission, and when condition changes.
  • Implement plan of care based on assessments and interdisciplinary goals.
  • Evaluate the effectiveness of the plan of care and revise as needed.
  • Coordinate labs, appointments, and special instructions for admissions and discharges.
  • Identify and interpret clinical changes related to medical diagnoses.
  • Question or clarify medical orders as necessary.
  • Carry out physician's orders in a timely manner.
  • Notify physicians of changes in resident conditions and follow through on actions taken.
  • Provide thorough reports to oncoming shifts and participate in walking rounds.
  • Assess and reassess pain, utilizing appropriate management techniques.
  • Educate residents and families regarding pain management.
  • Perform duties as a staff nurse when needed.
  • Submit written documentation in a timely and clear manner.
  • Keep leadership informed of issues or concerns promptly.
  • Follow through on resident concerns communicated during shift reports.
  • Coordinate with interdisciplinary care team members to implement and monitor care plans.
  • Address psychosocial, cultural, ethnic, and spiritual needs of residents and families.
  • Act as a preceptor/resource person for nursing staff.
  • Advocate for residents' rights and refer needs to the Ethics Committee as necessary.
  • Respond to or escalate complaints or investigations promptly.
  • Demonstrate creativity in problem-solving and innovative thinking.
  • Interact kindly and knowledgeably with residents, families, and staff.
  • Collaborate with Nurse Managers to achieve resident and facility goals.
  • Participate in the orientation of new staff and performance evaluations.
  • Follow proper infection control procedures at all times.
  • Remain calm in emergency or stressful situations.
  • Comply with regulatory requirements and MVHS policies.
  • Participate in performance improvement and continuous quality improvement activities.

Requirements

  • Two years' experience in a hospital, rehabilitation, or skilled nursing home.
  • Demonstrated competence in technical and manual skills as per RN scope of practice.
  • Good clinical assessment skills.
  • Ability to lead, guide, and motivate others.
  • Good interpersonal skills.
  • Basic computer knowledge.
