About the position

The RN Admission Nurse at Heavenly Gates Hospice Inc. is responsible for providing compassionate care to patients in the last phases of life-limiting illnesses. This role emphasizes the importance of comfort and dignity for patients, ensuring they receive individualized care that respects their unique needs and values. The nurse will assess patient needs, provide medical care, and educate families about hospice and palliative medicine, all while coordinating with other healthcare providers to ensure comprehensive care.

Responsibilities

• Provide emotional and spiritual support to patients and their families.
• Assess the physical, psychological, social, and spiritual needs of the patient and family.
• Provide end-of-life care and symptom management.
• Educate patients and families about hospice and palliative care services.
• Coordinate care with other healthcare providers.
• Participate in interdisciplinary team meetings to develop a plan of care for the patient.
• Document patient care activities in accordance with regulations and standards.

Requirements

• Registered Nurse (RN) license in California.
• Experience in hospice or palliative care preferred.
• Strong assessment and clinical skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Nice-to-haves

• Certification in hospice and palliative care nursing (CHPN).
• Bilingual in English and Spanish.
