About the position

The RN Admissions - Clinical Assessment Advisor plays a crucial role in overseeing the development and execution of patient care plans in a home health setting. This position involves direct interaction with patients, physicians, and care teams to ensure optimal clinical outcomes and adherence to healthcare quality initiatives. The advisor is responsible for evaluating patient needs, coordinating care, and ensuring effective communication among all stakeholders involved in the patient's care.

Responsibilities

  • Completes and oversees the development of the plan of treatment as approved by the physician.
  • Performs ongoing evaluation of patient needs and coordinates a team of professionals and home health aides.
  • Works with physicians and referral sources to obtain pertinent clinical information for assessments.
  • Ensures consistent scheduling and dissemination of updated clinical information among team members.
  • Coordinates assigned care team members and resources to achieve optimal outcomes.
  • Performs or delegates initial and ongoing evaluations of patient needs within their scope of practice.
  • Completes Start of Care/Resumption of Care/Recertification/Discharge of OASIS visits as needed.
  • Ensures the plan of care incorporates appropriate teaching related to health maintenance and safety.
  • Reassesses patient needs periodically and revises care plans as necessary.
  • Promotes communication between team members, physicians, and external case managers to ensure care coordination.
  • Identifies patient needs and assigns qualified caregivers to all cases.

Requirements

  • Bachelor of Science in Nursing (BSN) preferred.
  • At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role preferred.
  • Current CPR certification.
  • Excellent verbal and written communication skills.
  • Strong interpersonal skills.
  • Knowledge of state and federal home health agency regulations and compliance standards.
  • Ability to implement clinical policies and procedures.
  • Knowledge of clinical structure of PDGM.
  • Fluency in English, both written and spoken.
  • Good and regular attendance.
