About the position
Continuum Hospice is seeking a self-motivated and passionate Hospice RN Clinical Liaison to enhance outreach efforts to hospitals and medical facilities. This role focuses on identifying new referral sources and maintaining relationships to increase census and preference for hospice services. The RN Clinical Liaison will utilize clinical knowledge and empathetic communication skills to educate referral sources, families, and prospective patients about hospice services, ensuring extraordinary patient and customer service.
Responsibilities
- Determine primary decision makers and build positive relationships with the medical community.
- Promote hospice patient care services through regular visits to reflect the quality of care.
- Build the brand of the hospice as the preferred provider in the service area to achieve growth goals.
- Make regular calls to secure new referrals and meet established goals with referring physicians and healthcare providers.
- Conduct initial meetings with patients and families to overview hospice services and obtain consents for care.
Requirements
- Bachelor's degree in nursing or health care preferred.
- Current unencumbered license as a registered nurse.
- Minimum one year of sales/marketing experience in healthcare.
- One to two years' experience in hospice/palliative marketing strongly preferred.
- Skilled at establishing and maintaining working relationships with key staff and clinical staff.
