About the position

The RN Care Coordinator for Endocrinology is responsible for ensuring that patient and family needs are met during inpatient hospitalization or in outpatient settings. This role involves systematic data review, collaboration with multidisciplinary teams, and prioritization of clinical and psychosocial goals to enhance patient outcomes and family experiences.

Responsibilities

  • Systematically review data and collaborate with involved teams to meet patient and family needs.
  • Identify and prioritize clinical and psychosocial goals throughout the patient's stay.
  • Problem-solve to ensure follow-through with the multidisciplinary team.
  • Ensure optimal coordination and collaboration between teams, focusing on family experience and outcomes.
  • Complete ongoing staff education regarding coordination of care and culturally sensitive care.
  • Support unit-based and outpatient family activities to improve healthcare outcomes.
  • Document all relevant conversations, meetings, and referrals accurately.
  • Act as a liaison among clinical services and support services.
  • Participate in safe sleep rounds and initiate safety-based training as needed.
  • Send regular updates to the leadership team regarding family support and patient status.

Requirements

  • Bachelor of Science in Nursing (BSN) is required.
  • At least 3 years of inpatient/outpatient nursing experience is required.
  • Experience with care coordination and evidence of leadership within multidisciplinary teams is preferred.
  • Excellent communication skills are required.
  • Ability to work with diverse groups and de-escalate difficult situations is necessary.
  • Knowledge of clinical processes and hospital procedures for admission, inpatient stay, transfer, discharge, and outpatient settings is required.
  • Registered Nurse (RN) license is required.
  • Basic Life Support for Healthcare Provider (BLS) certification is required.

Nice-to-haves

  • Experience in Telehealth services is a plus.
  • Familiarity with evidence-based practice and research integration into care delivery.
