About the position

Healogics is the largest provider of advanced wound care services in the United States, treating more than 300,000 chronic wound patients annually across over 600 sites. The RN Care Coordinator plays a crucial role in the coordination of clinical activities within the Wound Care Center. This position is responsible for patient care assessment, planning, and implementation, as well as working on quality improvement initiatives to ensure optimal healing outcomes and patient satisfaction. The RN Care Coordinator collaborates closely with the Program Director and Medical Director to meet the Center's program objectives, ensuring compliance with all relevant policies and regulations, including those set by accrediting organizations and the Centers for Medicare and Medicaid Services (CMS). In this role, the RN Care Coordinator will be proficient in all responsibilities related to case management, which may include providing specific assessments according to established protocols and procedures. The RN will also provide clinical oversight to other clinical staff, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Medical Assistants (CMAs), and Hyperbaric Oxygen Technicians. The RN Care Coordinator is expected to engage in staff development, providing training and support to clinical staff, and to actively participate in identifying and implementing process improvements within the Center. The position may also involve coordination of vein care services if applicable, and the RN may function as a Documentation Assistant or perform hyperbaric safety director duties after completing the necessary training. Overall, the RN Care Coordinator is essential in ensuring that the Wound Care Center operates efficiently and effectively, providing high-quality care to patients.

Responsibilities

  • Coordinate clinical activities of the Wound Care Center.
  • Conduct patient care assessments and develop care plans.
  • Implement patient care strategies and monitor quality improvement initiatives.
  • Provide clinical oversight to nursing and support staff.
  • Collaborate with healthcare providers and hospital personnel regarding patient needs.
  • Engage in staff development and provide training and orientation to new staff.
  • Identify and implement process improvements in compliance with regulations.
  • Assist in the coordination of vein care services if applicable.
  • Function as a Documentation Assistant or hyperbaric safety director as needed.
  • Perform patient care based on staffing needs.

Requirements

  • Current RN (Registered Nurse) license required.
  • Associate's Degree in Nursing (ADN) minimum required; BSN preferred.
  • One year of experience as an RN required; two or more years preferred.
  • Prior supervisory experience preferred.
  • Wound care certification preferred.
  • Current BLS (Basic Life Support) certification required.
  • Strong planning, organization, and time-management skills.
  • Excellent interpersonal, oral, and written communication skills.
  • Ability to work collaboratively in a team environment.
  • Strong customer service and follow-up skills.
  • Proficiency in Microsoft Suite (Excel, Word, Outlook, PowerPoint) preferred.
