About the position

The RN Care Coordinator is a part-time role focused on managing patient care transitions in acute and emergency settings. This position involves assessing patient needs, developing care plans, and coordinating with healthcare teams to ensure safe and effective discharges. The RN Care Coordinator serves as a consultant and educator, optimizing resource utilization while maintaining high-quality patient care.

Responsibilities

  • Coordinates post-discharge patient care needs to assure timely and effective discharge of patients.
  • Completes and documents thorough patient assessments reflecting care needs and health plan benefits.
  • Communicates with patients, families, and healthcare teams to develop care plans that meet individual needs.
  • Facilitates resolution of issues that present barriers to safe transfers through patient/team care conferences.
  • Ensures ongoing contact with interdisciplinary staff to assure safe transitions across care settings.
  • Collaborates with healthcare teams to meet regulatory and compliance standards.

Requirements

  • Minimum of two years combined RN experience in Med/Surg, ICU, Emergency Department, Home Health, Skilled Nursing Facilities, Hospice, Long Term Acute Care, or Inpatient Rehab.
  • Successful completion of an RN program by date of hire.
  • High School Diploma or GED required.
  • Registered Nurse License (Oregon) within 6 months of hire.
  • Basic Life Support certification within 3 months of hire.

Nice-to-haves

  • Knowledge of appropriate utilization of acute hospital and Kaiser Permanente internal resources.
  • Knowledge of Medicare and Medicaid regulations related to eligibility requirements.
  • Knowledge of utilization management principles and tools.
  • Certified in Case Management.
  • BSN or bachelor's degree and MSN.
