About the position

The RN, Care Coordinator is responsible for assessing and identifying patient and family needs, coordinating care plans with the Interdisciplinary Group (IDG), and providing palliative and supportive care to patients and their families. This role emphasizes outstanding patient care and customer service, ensuring that the needs of terminally ill patients are met with compassion and professionalism.

Responsibilities

  • Assess and identify patient/family needs using the nursing process.
  • Coordinate the Plan of Care with the Interdisciplinary Group (IDG).
  • Provide palliative and supportive care to the patient/family unit.
  • Document patient/family contact information in the EMR and communicate with the IDG.
  • Complete clinical admissions for all Company services, explaining services to potential patients/families.
  • Educate patient/family on admission requirements and address immediate needs.
  • Initiate skilled nursing interventions to prevent complications and maximize comfort.
  • Develop strong relationships with case managers, physicians, and other healthcare professionals.
  • Provide care for GIP patients in a hospital, including daily visits and documentation.
  • Participate in Quality Assessment and Performance Improvement activities.

Requirements

  • Current RN license in the state of employment.
  • Minimum one year of in-patient nursing or hospice experience for external candidates; six months of LPN experience with hospice/home health for internal applicants.
  • Experience working with an EMR/EHR system.
  • Valid driver's license and automobile insurance.
  • Ability to communicate effectively in English (verbal and written).
  • Ability to manage emotional stress associated with caring for terminally ill patients.
  • Ability to physically access patient rooms and navigate various environments.

Nice-to-haves

  • Experience in hospice care.
  • BLS certification from the American Heart Association or Red Cross.
