About The Position
The RN Care Coordinator will be instrumental in implementing the Provider Partners Health Plan (PPHP) Model of Care (MOC) by engaging in assessment, planning, facilitation, care coordination, evaluation, and advocacy for members. This role focuses on promoting quality and cost-effective outcomes for individuals with complex and chronic conditions, ensuring comprehensive health needs are met through effective communication and resource utilization.
