About the position

The HIV/AIDS Care Manager RN at CVS Health is responsible for managing the care needs of Florida Medicaid members with a history of HIV/AIDS. This role involves driving care management and coordination activities, utilizing advanced clinical judgment to facilitate appropriate healthcare, and ensuring members receive comprehensive support for their medical and behavioral health needs. The position emphasizes a holistic approach to care, integrating various services to promote optimal health outcomes.

Responsibilities

• Conduct telephonic and/or face-to-face assessments of members' needs.
• Develop proactive care plans to address health issues and enhance overall wellness.
• Utilize clinical tools to perform comprehensive assessments and recommend case resolutions.
• Apply clinical judgment to reduce risk factors and address complex health indicators.
• Perform crisis intervention and follow-up for members in crisis situations.
• Serve as a single point of contact for members to address gaps in care.
• Collaborate with interdisciplinary care teams to achieve optimal outcomes.
• Utilize motivational interviewing skills to engage members in their care.
• Monitor and adjust care plans in collaboration with members and their care teams.
• Facilitate clinical hand-offs during transitions of care.

Requirements

• Active unrestricted RN license in Florida.
• Residence in the Orlando area.
• 2+ years of experience managing members with HIV/AIDS.
• 3+ years of clinical practice experience in a hospital or outpatient setting.
• 2+ years of experience using personal computers and MS Office Suite applications.
• Willingness to travel up to 40% in the Orlando area.
