About the position

The RN Care Manager at Methodist Hospital is responsible for providing patient-centered care management services within both the Emergency Center and inpatient settings. This role is integral to the interdisciplinary healthcare team, focusing on identifying care coordination needs and facilitating safe transition plans for patients and their families. The RN Care Manager will work collaboratively with various teams to address barriers to discharge and implement effective solutions, all while applying nursing values and promoting continuous quality improvement.

Responsibilities

• Provide patient-centered care management services to patients and families in the Emergency Center and inpatient settings.
• Identify care coordination needs in collaboration with inpatient and ambulatory care teams.
• Work with patients and their external support systems to secure appropriate transition plans.
• Participate in progressing patients towards discharge or the next level of care by identifying barriers and implementing solutions.
• Apply nursing values and continuous quality improvement in daily work.

Requirements

• Current Minnesota Registered Nurse (RN) license.
• Bachelor's Degree in Nursing strongly preferred; Associate's Degree in Nursing with commensurate experience considered.
• Minimum of one year hospital-based med-surg nursing experience preferred.
• Prior experience in utilization management, case management, home care setup, or discharge planning strongly preferred.
