About the position

The RN Care Manager at AdventHealth Shawnee Mission plays a crucial role in ensuring patient-centered care coordination and progression through the continuum of care. This position involves collaboration with patients, families, social workers, nurses, physicians, and the interdisciplinary team to facilitate efficient and cost-effective care. The RN Care Manager is responsible for evaluating post-hospital needs, developing transition of care plans, and implementing these plans prior to patient discharge. This role is essential for optimizing patient flow and enhancing continuity of care, ensuring patient satisfaction, safety, and preventing readmissions while managing the length of stay. The RN Care Manager communicates daily with the interdisciplinary team during multidisciplinary rounds, focusing on care coordination, discharge planning, and understanding medical necessity. This position requires the RN Care Manager to facilitate collaborative management of patient care across the continuum, intervening to remove barriers to timely and efficient care delivery. The RN Care Manager also provides education to the healthcare team on resource utilization, medical necessity, and compliance with CMS CoP for Discharge Planning. In addition to these responsibilities, the RN Care Manager is knowledgeable about post-hospital care services available to patients, including Home Health, Infusion Services, Durable Medical Equipment, Palliative Care, and more. The role requires adherence to departmental and system goals, policies, and procedures to ensure quality patient care and regulatory compliance. The RN Care Manager actively participates in outstanding customer service and maintains respectful relationships with all stakeholders.

Responsibilities

  • Ensure patient-centered care coordination and progression through the continuum of care.
  • Evaluate post-hospital needs and develop transition of care plans prior to patient discharge.
  • Communicate daily with the interdisciplinary team during multidisciplinary rounds.
  • Facilitate collaborative management of patient care across the continuum.
  • Provide education to nurses, physicians, and the interdisciplinary team on resource utilization and medical necessity.
  • Monitor patient flow and enhance continuity of care to prevent readmissions and manage length of stay.
  • Conduct initial evaluations for transition of care needs within one calendar day of admission.
  • Incorporate patient/family care goals and preferences into transition of care planning.
  • Identify and resolve barriers to timely and efficient care delivery.
  • Participate in daily multidisciplinary rounds to review patient progression and discharge plans.

Requirements

  • Associates Degree in Nursing or RN Diploma degree.
  • Registered Nurse (RN) license.
  • Two (2) years of medical/hospital nursing experience.

Nice-to-haves

  • Bachelor of Science in Nursing (BSN).
  • Health-related Master's degree or MSN.
  • Prior Care Management/Utilization Management experience.
