About the position

The RN Care Manager is responsible for the delivery of care coordination services for patients admitted to an inpatient bed. Provides oversight for the inpatient care team which includes the case manager assistant and other members responsible for assisting patients and families with navigating the pediatric system of care. Reviews the formulated plan of care to facilitate the patient's care to care providers post discharge. Works collaboratively with the interdisciplinary team to ensure care is delivered in an efficient and cost-effective manner. Promotes efficient use of health care resources and assumes a leadership role with the inpatient interdisciplinary team to achieve optimal clinical and resource outcomes. This position is at our Hematology Oncology clinic, and requires experience working with this patient population. The role is Monday-Friday days, but could involve rotating holiday coverage and/or weekend coverage if needed.

Responsibilities

  • Deliver care coordination services for patients admitted to an inpatient bed.
  • Provide oversight for the inpatient care team including case manager assistant.
  • Assist patients and families with navigating the pediatric system of care.
  • Review the formulated plan of care to facilitate post-discharge care.
  • Collaborate with the interdisciplinary team to ensure efficient and cost-effective care delivery.
  • Promote efficient use of health care resources.
  • Assume a leadership role with the inpatient interdisciplinary team.

Requirements

  • Minimum two (2) years of nursing experience in a pediatric acute care or ambulatory setting.
  • Bachelor's degree from an accredited university.
  • Basic Life Support (BLS); Ped Emerg Assess Recog & Stabil (PEARS) upon hire or must be obtained within six (6) months of hire with valid Pediatric Advanced Life Support (PALS); Registered Nurse - CA (RN).

Nice-to-haves

  • Three (3) years of nursing experience in an emergency department and/or case management, care coordination or transition of care.
  • Bachelor's or master's degree in healthcare related field from an accredited university.
  • Certification in Utilization Review and/or Case Management.
  • Knowledge of CCS/Medi-Cal regulations and requirements.
  • Bilingual in English/Spanish.
