About the position

The RN Care Manager is responsible for facilitating the discharge planning process from admission to discharge, ensuring quality patient care and safety while promoting effective transition planning and patient throughput. This role involves collaboration with the healthcare team to address complex psychosocial needs and optimize performance through best practices, all in alignment with Mercy's mission and values.

Responsibilities

• Facilitate the discharge planning process from admission to discharge in collaboration with the healthcare team.
• Intervene with patients who have complex or high-risk psychosocial needs as assigned.
• Promote integration of a seamless care model and coordinate patient throughput.
• Encourage healthcare team collaboration and coordination as evidenced by metrics.
• Optimize performance and adoption of best practices in care management.

Requirements

• Graduate of an accredited School of Nursing.
• Bachelor's Degree of Science in Nursing.
• Current License in the state of employment.
• 2-3 years acute care hospital setting experience preferred.
• Care Management or Utilization Management experience preferred.
• BLS (CPR) certification at hire date or within 90 days of hire.
• Certification in Case Management preferred.
• Strong verbal and written communication skills.
• Strong assessment, time management, and critical thinking skills.
• Advanced computer skills.

Nice-to-haves

• Knowledge of InterQual or Milliman Care Guidelines preferred.
