About the position

The RN Care Manager is responsible for performing comprehensive assessments of patients upon admission, ensuring that high-risk populations receive appropriate supportive services to prevent readmission. This role involves ongoing reassessment of patient needs throughout their hospital stay and coordination of care across various departments and facilities. The RN Care Manager also engages in utilization review activities and participates in performance improvement initiatives to enhance patient outcomes and satisfaction.

Responsibilities

• Perform initial comprehensive assessments on admission using established tools for high-risk indicators.
• Conduct ongoing reassessments of patient needs throughout the hospital course.
• Coordinate multidisciplinary care at both intra-hospital and inter-hospital levels.
• Manage transitional planning activities, including patient education and aftercare arrangements.
• Communicate relevant patient information to the healthcare team.
• Ensure department goals related to length of stay reduction, readmission prevention, and satisfaction improvement are met.
• Document patient information according to department requirements.
• Participate in performance improvement activities as needed.
• Perform utilization review activities per department policy.

Requirements

• Bachelor's degree in Nursing (BSN) or related field required.
• 5 years of clinical experience in emergency department, critical care, or medical/surgical settings required.
• Previous case management experience preferred.
• Knowledge of healthcare financial, regulatory, and payer issues preferred.
• Experience with Milliman/InterQual criteria is a plus.
• Valid Registered Nurse (RN) license in New York State required.
• PRI/Screen certification strongly preferred.
• Certified Case Manager (C.C.M.) certification preferred.
• Essential computer skills.

• Experience in surgery is a plus.
