About the position

The RN Care Manager position at Community Care is a full-time role focused on community case management for elderly individuals and adults with physical and intellectual disabilities. The role emphasizes team collaboration and aims to help members live independently in the community. The RN Care Manager will conduct assessments, develop care plans, and ensure the quality and effectiveness of services provided to members.

Responsibilities

• Completes discipline-specific initial and ongoing assessments.
• Conducts home visits and home assessments as needed, consulting with families and other supports for additional information.
• Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards identified outcomes, member strengths, risks, and supports.
• Follows up on all services to ensure quality, coordination, and effectiveness of services.
• Completes all required documentation and charting, assisting in the development and maintenance of member records.

Requirements

• Associates degree in Nursing required, Bachelor's degree preferred.
• Wisconsin Licensed Registered Nurse Licensure required.
• Eligibility for long-term care functional screener certification is required.
• WI Drivers License required.
• 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity required.
• 2 years of work in community nursing preferred.
