About the position

The Care Manager at Johns Hopkins Medicine Office of Population Health plays a crucial role in the primary care team, focusing on engaging and coordinating care for high-risk patients with complex healthcare and social needs. This position involves working closely with primary care providers to anticipate and manage care services across various settings, ensuring that patients receive comprehensive self-management and health education tailored to their chronic conditions or high-cost specialty care.

Responsibilities

  • Lead efforts to engage patients and coordinate care for high-risk and high-need patients.
  • Work alongside primary care providers to anticipate and coordinate care services across multiple providers and care settings.
  • Provide comprehensive self-management and health education for targeted member populations with chronic disease conditions or high-cost specialty care.
  • Demonstrate knowledge of complex care management core competencies, disease prognosis, and outcomes.
  • Ensure member goals are achieved within a specified time period based on individual needs and well-documented outcomes.

Requirements

  • Bachelor's Degree from an accredited school of nursing or Bachelor's Degree in social work.
  • Current registered nurse or social worker with licensure in the state of Maryland.
  • Minimum five (5) years of clinical practice experience.
  • Three (3) years of care management experience.
