Responsibilities

  • Provide consistent care coordination across the continuum, assessing the physical, psychological, and social needs of the patient.
  • Serve as an ongoing resource for patients throughout the care continuum.
  • Organize and facilitate clinical visit needs.
  • Assist patients and families by providing education, support, and information regarding available resources for assigned patient population within the Care Center and Southcoast community.
  • Provide educational materials and referrals to appropriate support groups and services.
  • Support patients and families to ensure compliance with care.
  • Create, monitor, and improve clinical quality indicators for program evaluation and any applicable regulatory or accreditation requirements.
  • Facilitate prospective and/or retrospective case conferences, documentation of conference outcomes including treatment recommendations for each patient.

Requirements

  • Graduate of an accredited School of Nursing or equivalent is required.
  • A Bachelor of Science (BSN) in Nursing or the equivalent education and experience is required.
  • Four (4) years of clinical experience with at least 2 years in a related setting is required.
  • Current RN Licensure in the Commonwealth of Massachusetts and/or Rhode Island is required.
  • Basic Life Support (BLS) certification is required.
  • Ability to multi-task, handle stressful situations in a fast-paced environment required.
  • Knowledge and clinical expertise in community services available to patients and their families required.

Nice-to-haves

  • Previous training as a care navigator, coordinator, or manager is highly desirable.
  • Previous ambulatory/outpatient experience is preferred.
  • Oncology experience is preferred.
