About the position

The RN Case Manager for the Cardiovascular ICU at Wake Forest Baptist Health is responsible for coordinating patient care, ensuring effective discharge planning, and collaborating with a multidisciplinary team to enhance patient outcomes. This full-time position operates during day shifts, Monday to Friday, and focuses on delivering compassionate care while managing the complexities of patient needs in a critical care environment.

Responsibilities

  • Identify patients who would benefit from Case Management interventions based on an initial screening assessment of discharge needs.
  • Assess all relevant data and obtain information by interviewing patient/family and performing objective evaluation of patient needs.
  • Develop and implement a Plan of Care in conjunction with nursing to enhance client outcomes in a cost-efficient manner.
  • Collaborate with the patient, caregivers, multidisciplinary team, and community service providers to effectively execute the Plan of Care.
  • Coordinate the discharge planning process, ensuring all needs are completed.
  • Evaluate and monitor the effectiveness of discharge planning and facilitate changes in the Plan of Care as needed.
  • Document Case Management actions in the Electronic Medical Record and communicate necessary information to providers and payors.
  • Participate in multi-disciplinary rounds to communicate discharge planning and drive patient progression through the system.
  • Establish partnerships with physicians to achieve institutional goals and contribute to excellent patient care.
  • Identify system issues and suggest interventions to eliminate duplication in services and document significant variances.
  • Identify trends/process barriers and participate in the development of plans to ensure effective patient movement through the care continuum.
  • Monitor medical necessity of admissions, continued hospitalization, and surgical procedures using criteria approved by the medical staff.
  • Utilize principles of quality improvement in performance by participating in unit and organizational quality improvement activities.
  • Provide age/developmentally-appropriate patient planning and communication in accordance with Age-Specific Care Guidelines.
  • Maintain working knowledge of payer and reimbursement practices impacting the plan of care.
  • Guide the patient and family through an evaluation of their options for post-discharge care.

Requirements

  • Graduation from an accredited School of Nursing; BSN required, MSN preferred.
  • Registered Nurse licensure from the State of North Carolina Board of Nursing.
  • Minimum 2 years of relevant clinical experience.
  • Previous Case Management experience preferred.
  • Basic Cardiac Life Support (BCLS) required.
  • Certification in Case Management highly recommended within 2 years of employment.

Nice-to-haves

  • Experience in a critical care environment.
  • Familiarity with Electronic Medical Records (EMR) systems.
