About the position

The RN Case Manager position involves assessing and evaluating members' needs to help them achieve optimal wellness. The role requires guiding members and their families towards appropriate resources for care and well-being, while managing various health issues. The Field Care Manager Nurse 2 will work independently, making decisions regarding care plans and monitoring patient progress, all while ensuring compliance with established guidelines and procedures.

Responsibilities

  • Assess and evaluate member's needs to achieve optimal wellness.
  • Guide members and families towards appropriate resources for care.
  • Manage physical, environmental, and psycho-social health issues.
  • Identify and resolve barriers to effective care.
  • Monitor patient care through assessments and evaluations.
  • Create member care plans as needed.
  • Make independent decisions regarding work methods and care strategies.

Requirements

  • Registered Nurse in Wisconsin with 2 years of experience in home case/care management.
  • Experience working with the adult population.
  • Knowledge of community health and social service agencies.
  • Ability to travel to member's residence within 30 to 40 miles.
  • Exceptional communication and interpersonal skills.
  • Ability to work with minimal supervision.
  • Proficiency in electronic information applications and software programs.
  • Intermediate to advanced computer skills, including Microsoft Word, Outlook, and Excel.
  • Ability to work a full-time flexible schedule.
  • Valid driver's license, car insurance, and access to an automobile.

Nice-to-haves

  • BSN
  • 3-5 years of in-home assessment and care coordination experience.
  • Experience with health promotion, coaching, and wellness.
  • Previous managed care experience.
  • Bilingual in English and Spanish.
  • Certification in Case Management.
  • Motivational Interviewing Certification.
