About the position

The RN Case Manager/Community Outreach role is a full-time position focused on providing comprehensive in-home care management services to high-risk patients. The primary goal is to prevent hospital readmissions and improve health outcomes by developing personalized care plans in collaboration with patients, families, and healthcare providers. This hybrid position requires travel within the DFW area and involves working closely with a multidisciplinary care team to ensure patients receive the necessary support and resources for effective condition management.

Responsibilities

• Conduct in-person visits to assess patient health status and needs.
• Generate comprehensive individualized care plans with patients and the care team.
• Provide education and support on managing chronic conditions and medications.
• Identify and address barriers to care and connect patients with community resources.
• Collaborate with healthcare team members to ensure coordinated care.
• Monitor patient response to care plans and revise as needed.
• Implement systems for monitoring high-risk patients to prevent acute exacerbations.
• Maintain documentation for all care management activities.
• Participate in case conferences and care coordination meetings.
• Engage in outreach initiatives to local provider and hospital groups.

Requirements

• Current Bachelor of Science in Nursing (BSN).
• Active RN license in good standing in TX, AZ, or IL.
• BLS certification.
• Current driver's license and reliable transportation.
• 2+ years of experience in adult and pediatric care settings.
• Strong clinical assessment and critical thinking skills.
• Excellent communication skills, both verbal and written.
• Ability to work independently and adapt to changing priorities.
• Empathy and ability to build rapport with diverse patients.

Nice-to-haves

• Current CCM Certification.
• 5+ years of nursing experience.
• Experience working with patients in their homes.
• Bilingual in Spanish and English.
• Comfortable with technology and remote work.
