About the position

The RN Case Manager at INTEGRIS Health is responsible for managing the clinical and financial outcomes for a designated caseload of patients. This full-time position involves coordinating patient care during hospitalization, ensuring optimal resource utilization, and facilitating efficient patient movement throughout the continuum of care. The role encompasses various functions including clinician, educator, researcher, manager, and advocate, all aimed at serving the best interests of both the healthcare system and the patients.

Responsibilities

  • Manage clinical and financial outcomes for an assigned caseload of patients.
  • Facilitate patient care during hospitalization by coordinating and monitoring resource utilization.
  • Proactively plan and negotiate efficient patient movement throughout the continuum of care.
  • Function as a clinician, educator, researcher, manager, and advocate for patients.
  • Adhere to National Patient Safety Goals based on the level of patient contact required.

Requirements

  • 5 years of experience in various clinical settings (e.g., home health, inpatient, physician office, clinic).
  • Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state.
  • Excellent interpersonal communication and collaboration skills.
  • Computer experience required, with a preference for Windows.
  • Current Driver's License issued by the state of Oklahoma.

Nice-to-haves

  • Experience with managed care and payer/provider requirements.
  • BSN preferred.
  • Current Case Management Certification preferred, to be attained within one year of hire (INTEGRIS Certified Case Management only).
