About the position

The RN Case Manager in the Emergency Department at Northwest Health - Porter Hospital is responsible for managing a caseload of patients to ensure optimal clinical, financial, and quality outcomes during their acute hospitalization. This role involves assessing the appropriateness of admissions and continued stays, monitoring care plans, and facilitating timely discharge planning in collaboration with patients, families, and the medical team.

Responsibilities

• Manage an assigned caseload of patients during acute hospitalization.
• Assess admission and continued stay appropriateness using clinical criteria and nursing skills.
• Monitor care plans to ensure timely delivery of care and effective discharge planning.
• Identify patients with post-discharge needs and collaborate with the medical team and payers to coordinate discharge plans.

Requirements

• Bachelor's degree in Nursing from an accredited college or university (preferred).
• Current Indiana Registered Nurse licensure (required).
• Previous experience in the Emergency Department (preferred).
