About the position

Responsibilities

• Coordinate care for the patient/family under the direction of the primary physician.
• Conduct assessments of patient/family needs.
• Develop the Interdisciplinary Group (IDG) Care Plan.
• Ensure implementation and coordination of interventions and services described in the care plan.
• Provide high-level personalized service to patients and families.
• Address all concerns promptly and effectively.

Requirements

• RN degree from an accredited college or university.
• Registered Nurse currently licensed in the State of Florida.
• Basic Life Support Certification.
• One year of experience in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or hospice nursing preferred.
• Basic computer skills and EMR software experience.

Nice-to-haves

• Hospice certification preferred.
• Bilingual (English/Spanish) preferred.
