About the position
The RN Case Manager position at Interim HealthCare involves managing a defined caseload of patients, providing direct patient care and comprehensive nursing services within the Greater Seacoast community. Responsibilities
- Manage a defined caseload of patients.
- Provide direct patient care and comprehensive nursing services.
- Coordinate with healthcare professionals to ensure quality care for patients.
- Document patient progress and maintain accurate records.
- Educate patients and their families on health management and care plans.
Requirements
- Valid Nursing License in the State of New Hampshire.
- Nursing experience required.
- Current CPR Certification required.
- Reliable transportation.
Nice-to-haves
- Experience in home health care or case management.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
