About the position

The Registered Nurse (RN) Case Manager at Blount Memorial Hospital is responsible for providing and directing skilled nursing care to home care patients. This role involves assessing patients' needs, coordinating with healthcare professionals, and ensuring effective communication to facilitate comprehensive patient care.

Responsibilities

• Provide and direct skilled nursing care to home care patients.
• Assess patients and their home situations.
• Communicate with physicians and other healthcare disciplines involved in patient care.
• Provide care and education to patients and their caregivers.
• Act as a liaison to facilitate patient care needs.

Requirements

• Licensed as a registered nurse in the State of Tennessee.
• Preferred RN experience of 2 years.
• Home care or emergency care experience preferred.
• Working knowledge of Medicare, Medicaid, and Joint Commission Standards preferred.
• Strong assessment and communication skills.
• Organizational and time management skills required.
• Self-directed and able to provide direct patient care.

Nice-to-haves

• Experience in home care or emergency care settings.
• Knowledge of State of Tennessee healthcare regulations.
