About the position
The RN Case Manager for Home Health is responsible for administering skilled nursing care to patients in their homes, following physician orders and care plans. This role involves patient and family education, communication with healthcare providers, and supervision of other nursing staff. The RN will also participate in quality assurance programs and interdisciplinary assessments, ensuring high-quality care delivery.
Responsibilities
- Administer skilled nursing care to patients in their homes according to physician orders.
- Participate in patient, caregiver, and family counseling and education.
- Communicate with physicians and healthcare practitioners regarding the plan of care.
- Provide services as ordered by the physician in the plan of care.
- Supervise the care provided by Licensed Vocational Nurses and Home Health Aides.
- Participate in the development and revision of the plan of care in collaboration with patients and caregivers.
- Contribute to quality assurance and performance improvement programs.
Requirements
- Currently licensed as a Registered Nurse in the state of practice.
- Two years of nursing experience, preferably in home health.
- CPR certified by the American Heart Association or American Red Cross.
- Strong communication skills, both oral and written.
- Ability to exercise initiative and independent judgment.
- Valid driver's license and reliable transportation.
Nice-to-haves
- Experience in case management (1 year preferred).
- Experience in home health (1 year preferred).
- Ability to relocate to South Houston, TX.
