About the position

Clover Home Health is seeking a dedicated Registered Nurse (RN) for a part-time, hybrid position to provide home health care to clients in the Greater Indianapolis area. The role requires a compassionate approach to nursing, with a focus on delivering high-quality care and maintaining strong communication with clients and the healthcare team. The ideal candidate will have significant experience in home health nursing and proficiency in OASIS charting and EMR systems, particularly AXXESS.

Responsibilities

• Complete new patient intakes
• Direct patient care including SOC's, ROC, Re-Certs, Discharge, Transfers, Nurse Visits, and all OASIS documentation
• Document complex medical information to improve health outcomes
• See patients and clients as needed
• Work collaboratively with the Director of Nursing (DON) and Assistant Director of Nursing (ADON)
• Communicate patient conditions and collaborate with the DON/ADON to adapt care as needed
• Promote preventative care to decrease re-admissions and ensure continuity of care
• Perform additional duties as assigned

Requirements

• Current nursing license for the state of Indiana
• Degree in Nursing from an accredited institution
• Valid driver's license and automobile insurance
• Current in-person CPR certification
• TB test screening within the past 12 months
• Minimum of 3 years of home health nursing experience
• Minimum of 2 years of OASIS experience
• Tech-savvy with the ability to navigate multiple platforms
• Proficient with Apple products
• Excellent communication and listening skills
• Strong time management skills
