About the position

The RN - Case Manager for Home Health position involves providing and managing oversight of patients receiving home care services in various counties including Wayne, Oakland, and Macomb. The role requires the nurse to conduct home visits, educate patients on disease management and medications, and assist in directing the care team to meet each patient's unique needs under the guidance of the Director of Clinical Services. Candidates must be autonomous, timely, and possess excellent communication skills, with a minimum of two years nursing experience, including one year in community nursing care.

Responsibilities

Requirements
