About the position

The Registered Nurse Case Manager (RNCM) at Brookhaven Hospice is a full-time role focused on providing compassionate end-of-life care to patients and their families. The RNCM will utilize the nursing process to assess, plan, implement, and evaluate hospice care, ensuring high standards of professional nursing and clinical competency. This position requires collaboration with interdisciplinary team members and involves direct patient care, education, and advocacy.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with compassion.
• Partner closely with the Clinical Director to assess client eligibility for hospice services.
• Provide direct care and support to patients, ensuring their comfort and quality of life.
• Deliver nursing care rooted in proven principles and techniques to ease pain and provide solace.
• Educate and empower patients and families to participate in creating personalized care plans.
• Share responsibility for call duties, demonstrating commitment to patient care.

Requirements

• Current unencumbered license as a Registered Nurse (RN).
• Minimum one year of experience as an RN in a medical-surgical or acute care setting.
• Experience in hospice care is a plus.
• Proficient in typing and various computer programs, including EMR systems.
• Current CPR certification.
