About the position

The PRN Registered Nurse Case Manager (RNCM) at Lighthouse Hospice is responsible for providing compassionate and high-quality end-of-life care to patients and their families in the Mercer County area. This role involves planning, organizing, and directing hospice care while effectively interacting with patients, caregivers, and interdisciplinary team members. The RNCM will ensure that care standards are maintained and will play a crucial role in enhancing the quality of life for patients during their final stages of life.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Partner closely with the Clinical Director to assess the eligibility and suitability of clients for hospice services.
• Provide direct care to patients, ensuring their comfort and quality of life are prioritized.
• Deliver nursing care rooted in proven principles and techniques to ease pain and provide solace.
• Educate and empower patients and families to participate in creating personalized care plans.
• Share the responsibility of call duties, demonstrating commitment to being available when it matters most.

Requirements

• Current unencumbered license as a Registered Nurse (RN).
• Minimum one year of experience as an RN in a medical-surgical or acute care setting.
• Experience in hospice care is a plus.
• Proficient in typing and various computer programs, including EMR systems.
• Current CPR certification as required by state.
