We are hiring for an RN Case Manager in Hospice at Heart n Home Hospice of Baker City, part of LHC Group. As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. This includes making the initial nursing evaluation to determine eligibility for hospice services within forty-eight (48) hours of referral. You will identify the physical, psychosocial, and environmental needs of the patient and their family, reassessing these needs no less than every fifteen (15) days. You will initiate and coordinate the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition. Your documentation will include problems, appropriate goals, interventions, and the patient/family response to hospice care. Collaboration is key in this role, as you will work closely with the patient/family, attending physician, and other members of the Interdisciplinary Group (IDG) to provide comprehensive care daily.

Responsibilities

  • Collaborate with the patient/family, attending physician, and other members of the IDG in providing patient and family care daily.

Requirements

  • Current RN licensure in the state of practice.
  • One year of clinical experience.
  • Current CPR Certification.
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
