About the position

The RN Case Manager for Hospice is responsible for coordinating all aspects of patient care in accordance with professional standards and practices. This role involves working closely with an interdisciplinary team to assess patient needs, develop care plans, and provide supportive care to patients and their families. The position requires travel to patient homes, ensuring effective communication and collaboration with healthcare providers and family members to maximize patient comfort and health.

Responsibilities

  • Coordinate all aspects of patient care in accordance with professional standards.
  • Identify patient/family needs and provide supportive care as per physician's orders.
  • Participate in ongoing comprehensive assessments and case conferences.
  • Supervise and direct others in planning and implementing patient care.
  • Maintain regular communication with supervisors and attending physicians regarding patient status.
  • Provide 24/7 coverage to patients/families, including on-call responsibilities.
  • Assess, plan, implement, and evaluate care for hospice patients in various settings.
  • Develop individualized care plans in collaboration with the interdisciplinary team.
  • Document patient records accurately and maintain up-to-date information.
  • Provide clinical supervision to ancillary staff and volunteers.

Requirements

  • Registered Nurse, currently licensed in Massachusetts in good standing.
  • Associate's degree in nursing required; Bachelor's degree preferred.
  • Minimum three years of varied work experience as a professional nurse, with one year in an acute setting preferred.
  • Demonstrated ability to assess and respond to patient and family needs.
  • Skilled in nursing practice and able to cope with family emotional stress.
  • Understanding of hospice philosophy and principles.
  • Ability to be self-directed, flexible, and cooperative in a team environment.
  • Must have a reliable vehicle and valid driver's license.

Nice-to-haves

  • Experience in hospice or palliative care settings.
  • Certification in hospice and palliative nursing.
