About the position

The Hospice Registered Nurse Case Manager is responsible for providing high-quality nursing care to patients in a hospice setting. This role involves managing patient cases, conducting home visits, and collaborating with interdisciplinary teams to ensure comprehensive care for patients and their families. The position requires strong clinical skills, effective communication, and the ability to work independently in a home health environment.

Responsibilities

• Manage patient cases and develop individualized care plans.
• Conduct home visits to assess patient needs and provide nursing care.
• Collaborate with interdisciplinary teams to coordinate patient care.
• Educate patients and families about hospice services and care options.
• Document patient progress and maintain accurate medical records.
• Ensure compliance with healthcare regulations and standards.

Requirements

• Graduate of an accredited school of nursing.
• Current RN licensure in the state of California.
• Minimum of 1-2 years of recent acute care experience in a clinical setting.
• 1-2 years of experience in home health or hospice care preferred.
• Basic Life Support (BLS) Hands-On CPR certification.
• Strong observational, communication, and problem-solving skills.
• Valid driver's license and reliable vehicle for travel.
