About the position

The Registered Nurse Case Manager (RNCM) at Altus Hospice is responsible for planning, organizing, and directing hospice care for patients and their families in the Corpus Christi area. This role emphasizes compassionate care, effective communication with interdisciplinary teams, and maintaining high standards of professional nursing. The RNCM will engage with patients and families to ensure comfort and quality of life, while also collaborating with the Clinical Director to assess client eligibility for hospice services.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with compassion.
• Partner closely with the Clinical Director to assess the eligibility and suitability of clients for hospice services.
• Provide direct care to patients, ensuring their comfort and quality of life.
• Deliver nursing care rooted in proven principles and techniques to ease pain and provide solace.
• Educate and empower patients and families to participate in creating personalized care plans.
• Share responsibility for call duties, demonstrating commitment to patient care.

Requirements

• Current unencumbered license as a Registered Nurse (RN).
• Minimum one year of experience as an RN in a medical-surgical or acute care setting.
• Proficiency in typing and various computer programs, including EMR systems.
• Current CPR certification as required by state.
