About the position

The RN Case Manager (Hospice) position at Harbor Healthcare System is a full-time role focused on providing comprehensive care to patients in an outpatient setting. Responsibilities

• Admit patients and coordinate appropriate care for each patient and family.
• Conduct assessments and manage patient discharges.
• Perform pronouncements as required.
• Communicate effectively with patients, families, and the interdisciplinary care team.

Requirements

• Licensed RN in the State of Texas - required
• 1+ years' experience as a RN - preferred
• Experience with Hospice or Home Health - preferred
• Effective written and verbal communication skills
• Ability to travel and have reliable transportation
• Clean background, drug screen, and driving record.
