About the position
The RN Case Manager (Hospice) position is a full-time role based in Rockport, TX, requiring an experienced Registered Nurse to join the interdisciplinary care team. The primary responsibility of the RN Case Manager is to admit and coordinate care for patients and their families, ensuring appropriate services are provided throughout the care process.
Responsibilities
- Admit patients and coordinate their care
- Conduct assessments and discharges
- Perform pronouncements as required
- Communicate effectively with patients and families
- Collaborate with the interdisciplinary care team
Requirements
- Licensed RN in the State of Texas
- 1+ years' experience as a RN
- Experience with Hospice or Home Health preferred
- Effective written and verbal communication skills
- Ability to travel and have reliable transportation
- Clean background, drug screen, and driving record
